Privacy Statement

This notice explains what information I collect, and how I use it. I will process personal data about you and I recognise the need to treat it in an appropriate and lawful manner. The purpose of this notice is to make you aware of how I will handle your information. 

My role regarding your data

I take the issue of security and data protection very seriously and strictly adhere to guidelines published in the General Data Protection Regulation (EU) 2016/679 which is applicable from the 25th May 2018, together with any domestic laws subsequently enacted. I am notified as a Data Controller with the Office of the Information Commissioner under registration number: ZA675968

I am required to let you know that I hold the following personal information relating to you:

·     Name, Date of Birth, Address, Telephone Number, Email address, GP Details

I hold this information to enable me to undertake and perform my obligations and duties to you in accordance with my role, and to communicate with you during the course of our work together.

The information is stored as a paper record and kept in a locked filing cabinet. I may also write brief hand-written session notes where appropriate and these too are kept in a locked filing cabinet. The records are retained by me whilst our work is current and destroyed 5 years from the date of completion of our work together.

Sharing your information with third parties

I do not share your information with any third parties except in the following situations:

·     I may share your information with professionals or others at your request.

·     I may share your information with relevant professionals or authorities if I believed that you or another person was at risk of serious harm.

·     I may share your information where required to do so by law.

·     In the above circumstances I would always endeavour to acquire your specific consent.

·     I will share your name and contact details with my Clinical Executor so they may contact you in the event of my death or incapacity.

·     I will share your name and details of bank transfer payments with my administrator to ensure the smooth operation of my business.

By signing a contract and agreeing to work with me, you are agreeing to the terms as laid out in this Privacy Statement.
